Service Fees

The initial evaluation is approximately 60 minutes in length and the fee is $120. 

Individual sessions are based on a 45 minute clinical hour and the fee is $100.  If a longer hour session is desired,
the fee is $120.

Psychological evaluations for couples relating to fertility issues are 60 minutes and the fee is $250.  This fee includes the report that is provided to your fertility clinic.

Fees are collected at the time of service.  Payments are accepted by check, cash, HSA cards, debit cards, VISA, MasterCard or Discover.

For those insurance companies indicated as non-participating, we will file an insurance claim for you as a courtesy.  This may enable you to receive reimbursement based on the specifications of your insurance policy.  Our fees are at or below what is considered reasonable and customary.


If you are unable to make your scheduled appointment, please contact our office 24 hours in advance. Missed appointments, those without proper notice of cancellation or need for rescheduling, will be assessed a $80 fee. Please be aware that insurance companies will not reimburse for missed appointments.

Arrival Time Policy

Please note that if you arrive late for your appointment, you are forfeiting that time. Your appointment starts at the time on your appointment card, not when you arrive.

Health Insurance

You need to contact your insurance company in advance to verify if you need a referral for services, get an authorization number and check your benefits including deductibles, co-pay and number of visits available.  There are several types of insurance plans (HMO, PPO, POS, etc.) each with different requirements for paying claims.  It should be noted that authorization is not a guarantee of payment and you are still ultimately responsible for all charges.
