Service Fees
The initial evaluation is approximately 60 minutes in length and the fee is
$135.
Individual 45 minute session is $115.
Family and One hour long sessions are $135.
Fees are collected at the time of service. Payments are
accepted by check, cash, HSA cards, debit cards and all major credit cards.

For those insurance companies indicated as
non-participating, we will file an insurance claim for you as a
courtesy. This may enable you to receive
reimbursement based on the specifications of your insurance
policy. Our fees are at or below what is considered
reasonable and customary.
Cancellation/Rescheduling
If you are unable to make your scheduled
appointment, please contact our office 24 hours
in advance. Missed appointments, those without
proper notice of cancellation or need for
rescheduling, will be assessed a $85 fee. Please be aware that insurance companies will
not reimburse for missed appointments.
Arrival Time Policy
Please note that if you arrive
late for your appointment, you are forfeiting that time. Your
appointment starts at the time on your appointment card, not when
you arrive.
Health Insurance
You need to contact your insurance company in advance to verify if
you need a referral for services, get an authorization number and
check your benefits including deductibles, co-pay and number of
visits available. There are
several types of insurance plans (HMO, PPO, POS, etc.) each with
different requirements for paying claims.
It should be noted that authorization is not a guarantee of payment
and you are still ultimately responsible for all charges.
